Summary
Provides street outreach and housing navigation services to unhoused individuals and families, administers the Community Screening Tool, and provides linkages to community resources.
Assist in the fulfillment of the Agency’s vision, “A strong Fresno County where people have resources to shape their future free from poverty.”
Duties And Responsibilities/Essential Functions
Administers the Community Screening Tool and/or other assessment tools.
Provides advocacy, linkages, transportation and support to clients while assisting them with access to basic needs and mainstream services identified through assessment. Transportation includes driving a company vehicle to transport unhoused clients to appointments and includes driving as part of outreach in Fresno and Madera counties to identify unhoused individuals and families.
Provides navigation services to unhoused individuals and families including shelter placement.
Monitors clients progress to assist with any identified barriers preventing them from accessing resources.
Identifies available community resources that provide housing, food, clothing, documentation assistance, legal services, mental health services, addiction services, faith-based services, as well as identifying people in their community that they can access for assistance.
Maintains excellent working relationships with community organizations, treatment providers and collaborative partners and have strong client engagement skills.
Collects and documents data as required, and assists in compiling statistical data, Homeless Management Information Systems (HMIS), and monthly reports.
Maintains clear, sufficient documentation on all program clients.
Adheres to the confidentiality policy regarding clients.
Collaborates with all Sanctuary and Support Services programs, Fresno EOC programs, and partner agencies to ensure success of clients.
Attends staff meetings and trainings as required.
Completes any child abuse referrals as needed, as required of all mandated reporters.
Adheres to all guidelines, policies, procedures, and protocols of the Sanctuary and Support Services Program and Fresno Economic Opportunities Commission.
Travels for agency business using reliable transportation.
Maintains regular attendance and punctuality.
Works evenings and weekends as required.
Performs other duties as assigned.
Qualifications

Education
- High school diploma or GED.
- Minimum of 60 college units in a Human Services field required. Work experience may substitute required education on a year-by-year basis.
Experience
- Minimum of one (1) year related experience required working with behavioral health, homeless populations or in a social service related area required.
Other Qualifications
Basic knowledge of the Welfare and Institutions Code and other laws affecting children and families in the State of California preferred.
Basic knowledge of Community Care Licensing, Title 22 Group Home standards for facilities, staffing, and services preferred.
Familiarity and ability to work with youth/families of all cultural, ethnic, religious and socio-economic backgrounds.
Proficient in Microsoft Office Suite, related business software and web-based reporting systems.
Willing to work non-traditional hours and days to meet the needs of this position.
Effective presentation, written and verbal communication skills.
Bilingual English and Spanish or other secondary language preferred based on the needs of the clients and the program.
Pass required background investigation including medical examination and TB clearance, acceptable criminal record statement, Department of Justice fingerprint clearance, and child abuse index clearance.
Maintain valid California driver's license and the minimum auto insurance as required by state law, or reliable transportation. For those driving agency vehicles, must maintain insurability under agency policy.
CPR and First Aid training preferred; required within six (6) months of hire.

While performing the duties of this job, the employee is regularly exposed to outside weather including extreme conditions. The employee is regularly exposed to transient populations and higher crime risk areas. The employee is regularly exposed to and required to walk on rough and uneven terrain. The noise level in the work environment is usually moderate.
